Klerksdorp Midweek, Klerksdorp - Covid-19 vaccination for children aged five and eleven years is available at Klerksdorp/Tshepong Hospital Complex.

Parents or guardians are encouraged to bring their children with chronic diseases for vaccination. 

The vaccination site is at Kampong in Klerksdorp Hospital. Operating hours are Monday to Friday from 07:30 - 16:00.

Any child who is receiving chronic treatment is welcome, irrespective of the centre where the child is being treated. There is no need to make an appointment or a booking in advance.

Please bring the following for the child to be vaccinated. 

Child’s birth certificate, passport or asylum document, parent or guardian’s proof of identity (identity card/ passport/ asylum), health care professional’s referral letter even when the child is being followed up at Klerksdorp Hospital. 

The following information should appear on the referral letter: Name of the facility, name and date of birth of the child, name of the healthcare professional and contact number and chronic diseases and treatment prescribed for the child.
